Athletes and families must review, sign, and submit the following documents and information on the Milwaukee City Conference Student-Athlete Registration Portal<\/a>:<\/p>\n

- Family and student info: Name, address, chosen activity, emergency contact<\/li>\n

- Proof of Insurance (pic of front\/back of card); option to purchase from K&K Insurance for sports season if needed<\/li>\n
- Student-Athlete Handbook<\/li>\n
- Wisconsin Interscholastic Athletic Association eligibility forms<\/li>\n<\/ul>\n
Milwaukee Public Schools has partnered with Concentra health care services<\/a> to provide physical exams for high-school athletes, or you may work with your child\u2019s current medical provider. Athletes may participate in only one sport per season. There are three seasons each school year: fall, winter, spring.<\/p>\n
