The inability to pay fees or to receive Wisconsin Shares authorization cannot be a barrier to participation for students.<\/p>\n For additional fee information, please contact the Forest Home Avenue School Safe Place coordinator at (414) 902-6288.<\/p>\n Parents\/guardians can complete Safe Place Youth Participant Registration and Emergency & Health History forms in advance, or get copies on-site. Completed forms must be submitted to the Forest Home Avenue School Safe Place.
