Advice for High School Athletes

Student-athletes are expected to attend practice every day and stay on top of academics. Students should notify their coach ahead of time if they cannot make a practice.

Student-Athlete Registration

Athletes and families must review, sign, and submit the following documents and information on the Milwaukee City Conference Student-Athlete Registration Portal:

  • Athletic fee
  • Athletic permit card
  • Code of Conduct
  • Concussion Agreement forms
  • Family and student info: Name, address, chosen activity, emergency contact
  • Physical exam form, completed by medical professional
  • Proof of Insurance (pic of front/back of card); option to purchase from K&K Insurance for sports season if needed
  • Student-Athlete Handbook
  • Wisconsin Interscholastic Athletic Association eligibility forms

Milwaukee Public Schools has partnered with Concentra health care services to provide physical exams for high-school athletes, or you may work with your child’s current medical provider. Athletes may participate in only one sport per season. There are three seasons each school year: fall, winter, spring.
