Immunization Information<\/h2>\r\nChildren, especially young ones, are more likely to get very sick from diseases that can be prevented by immunizations. To be fully protected from these diseases, children may need more than one vaccination. For this reason, it takes about seven trips to the doctor to make sure your child has all the recommended shots. Immunizations are required for entry into childcare and school so it is important that you make sure your child is up-to-date.\r\n\r\nParents are responsible for providing the school or child care facility with an up to date copy of their child\u2019s immunization record. Immunization records can be obtained from your medical provider or from the City of Milwaukee Health Department by calling 286-8034. Medical waivers are available for those children who are not medically or otherwise able to be vaccinated. A signature from your child\u2019s health care provider on the waiver form is sufficient for medical waivers, otherwise a parent may sign if it is for religious or personal convictions.\r\n\r\nMPS nurses review student immunization information in our student database system as well as in the Wisconsin Immunization Registry (WIR) throughout the school year. Nurses are available on a daily basis to provide families with immunization information, education, and resources for their child\u2019s specific needs.\r\n\r\nPlease contact your school to share updated immunization information and if you would like to speak with\u00a0your school\u2019s nurse.\r\n\r\nAttached are some resources related to where to obtain vaccines as well as immunization questions you may have.
