Registration
Please help your child complete the electronic registration process for South Division athletics by signing and submitting the following documents and information on the Milwaukee City Conference Student-Athlete Registration Portal:
- Athletic fee
- Athletic permit card
- Code of Conduct
- Concussion Agreement forms
- Family and student info: Name, address, chosen activity, emergency contact
- Physical exam form, completed by medical professional
- Proof of Insurance (pic of front/back of card); option to purchase from K&K Insurance for sports season if needed

- Wisconsin Interscholastic Athletic Association eligibility forms
Milwaukee Public Schools has partnered with Concentra health care services to provide physical exams for high-school athletes, or you may work with your child’s current medical provider. Athletes may participate in only one sport per season. There are three seasons each school year: fall, winter, spring.
Game Times and Tickets
Cheer us on, anytime. The Cardinals and Lady Cardinals love your support. Tickets for all athletic events must be purchased online and in advance.
Advice for High School Athletes
In high school, student-athletes are expected to attend practice every day and stay on top of academics. Students should notify their coach ahead of time if they cannot make a practice.
